Collection of Personal Health Information

We collect personal health information about you directly from you or from the person acting on your behalf.

The personal health information that we collect may include, for example, your name, date of birth, address, health card number, health history, records of your visits to Dialysis Management Clinics Inc., and the services that you received during those visits. We also on occasion collect personal health information about you from other sources within your circle of care or if the law permits. We collect this information directly from the provider within your circle of care or by the use of shared electronic systems.  

Uses and disclosures of Personal Health Information

We use and disclose your personal health information to:

  • Treat and care for you
  • Your obvious circle of care (referring renal program, transplant program, family physician and or community agency / specialists)
  • Administer and manage our internal operations
  • Receive remuneration for your treatment and care (from OHIP and others if required)
  • Conduct risk management activities
  • Conduct quality improvement activities (such as sending patients satisfaction surveys)
  • Teach
  • Compile statistics
  • Comply with legal and regulatory requirements
  • Fulfil other purposes permitted or required by law

Who has access to your records

All clinical and administrative staff involved in either the use and or disclose of your personal health information have access to the personal health information we collect at Dialysis Management Clinics.

Your Choices

You may access and correct your personal health records, or withdraw your consent for some of the above uses and disclosures by contacting us (subject to legal exceptions).

Important Information

We take steps to protect your personal health information from theft, loss and unauthorized access, copying, modification, use, disclosure and disposal. We monitor and manage our privacy compliance. We take steps to ensure that everyone who performs services for us protects your privacy and only uses your personal health information for the purposes you have consented to.
