Our Team

Primary Care Team

At the core of who we are is a small dedicated primary care team that, in their own words, are driven by a shared purpose to celebrate life and provide the best care. This group of primarily registered nurses and support staff foster an environment that is uniquely supportive, welcoming and adaptable. By collaborating with care teams from our referring hospital programs, including allied health members, we are able to respond to the needs of our communities and ensure equitable access to care for all our patients and families.

Leadership Team

Developing leadership across our organization has always been a core focus for us. Providing opportunities for growth and development have shaped us into who we are; and while all team members are leaders, a select few are entrusted to foster and maintain the environment that makes our space special. We are proud to have a leadership team that grew from our primary care team and the therefore understands the importance of staying true to our principles and values.

Nephrologist Advisors and Directors

We draw on the collective expertise of upwards of 40 Nephrologists that support patients at DMC, each of whom act as an advisor and resource to our team in delivering care. Amongst our Nephrologist advisors, we have designated Medical Directors, Quality and Committee Advisors. These roles provide leadership in overseeing medical policies for the facilities, supervising the review of patient care and acting as a site resource. They also ensure the services provided at each site are in alignment with legislative and clinical requirements, are members / chair our quality assurance committee and personally attend the facilities.

Hospital Programs

Our partner hospital programs work with us in close collaboration to support the care of our shared patients. After facilitating a seamless transition of care, hospital programs maintain their involvement in care through wrap around services and access to as needed hospital care. While many hospital renal programs have patients dialyzing at one of our sites, the following are the core programs we support:

The Kidney Foundation

The Kidney Foundation offers our patients educational resources, short term financial assistance and peer support services.

Trillium Gift of Life Network

Trillium Gift of Life Network and Transplant Programs help in outlining the requirements to ensure eligible patients have access to transplant as an option in their care.

 

The Transplant Ambassador Program

The Transplant Ambassador Program offers recipients or living kidney donors to share their personal experiences with our patients and family members.

Ontario Renal Network

The Ontario Renal Network (ORN) manages the delivery of chronic kidney disease services across the province of Ontario. The ORN allocates resources, supports and performance requirements for our three publically funded sites. Guiding their work is the Ontario Renal Plan, a provincial strategic road map to help improve the lives of people at risk for and living with chronic kidney disease.

Clinical Trials Ontario

Clinical Trials Ontario streamlines the processes to help make enable us to participate in high-quality clinical trials for kidney patients.

The Mohawk Medbuy

Mohawk Medbuy is a national not-for-profit shared services organization that helps us find efficiencies and cost savings on the supplies we use in our care.
